I'm leaving for college the day after our anniversary. I know its not something more common like 6 months, but I'll be seeing her only once a week; its 1h45m away. So I wanted to celebrate it well with my sweetie.

i wouldn't call it a 5 month anniversary date. doing special things for the heck of it are so much nicer. just go get some food from grocery and have a picnic at central park or another park. sit on the benches and people watch. just let her know that you are going to miss her and her pretty face and be cuddly. have fun.5 Month Anniversary Date ideas?

there is no such thing as a 5 month anniversary. anniversaries, are by definition annual events. do not celebrate anything that does not exist.
